The program should follow legal guidelines, use accurate health data, and communicate expectations clearly. When designed well, these programs drive measurable behavior change, improve employee health outcomes, and help control long-term healthcare costs.<\/p>\n<\/div><\/div>\n<\/blockquote>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-what-is-a-health-contingent-wellness-program\"><span class=\"ez-toc-section\" id=\"What_is_a_health-contingent_wellness_program\"><\/span>What is a health-contingent wellness program?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>Health-contingent wellness programs is a wellness incentive program based on health risks. A health-contingent wellness program is one in which employees must A) meet a health standard or B) meet a <a href=\"https:\/\/www.dol.gov\/sites\/dolgov\/files\/ebsa\/about-ebsa\/our-activities\/resource-center\/publications\/caghipaaandaca.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">reasonable alternative standard<\/a> in order to earn an incentive or avoid a penalty. In most cases, a health factor is just a biometric value like healthy blood pressure or cholesterol, or not using tobacco.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\" id=\"block-e21b2b74-ed59-4f79-ac9d-57466b1375ee\"><span class=\"ez-toc-section\" id=\"No_More_Sharing_Risk\"><\/span>No More Sharing Risk<span class=\"ez-toc-section-end\"><\/span><\/h3>\n\n\n\n<p id=\"block-1c30e3d9-916c-4c39-b63c-4130063af402\">The cost of health care is calculated differently than the cost of life insurance, house insurance, or car insurance. Unsafe driving leads to higher car insurance.<\/p>\n\n\n\n<p id=\"block-17974146-872c-4911-934d-6c501ac0acce\">If an employee is big into BASE jumping, their life insurance will be higher because the premiums are based on risk of premature death. Having a fireplace or flooding potential or asbestos siding are all risks that will increase the cost of homeowners insurance.<\/p>\n\n\n\n<figure class=\"wp-block-image\" id=\"block-29540489-13b1-47eb-a9a5-d2d9bd18b16e\"><img decoding=\"async\" src=\"https:\/\/www.wellsteps.com\/blog\/wp-content\/uploads\/2016\/02\/base-jump-1024x678.jpg\" alt=\"high risk lifestyles\"\/><\/figure>\n\n\n\n<p id=\"block-0437d2fc-f2b5-4654-8742-84de9e839f92\">The cost of insurance is calculated on the amount of risk the insurance company is taking. But for decades, the cost of health care has been unrelated to individual health risks.<\/p>\n\n\n\n<p id=\"block-d612320f-03b8-44a6-9db8-62f7b179086b\">However, now your health insurance is beginning to take the same risk-based approach to pricing as other forms of insurance. But there is one important twist; risk assessment with health insurance is usually voluntary and not forced upon you by the insurance company. A health-contingent (outcomes-based) wellness program is a step in that direction.<\/p>\n\n\n\n<p id=\"block-403f0771-37fb-4485-b4ae-39ee159e4fa9\">Incentives can be offered to those who have lower risks or who are working to improve their health. Those who maintain their health, and who are therefore less risky to insure, get a better deal on health insurance through lower deductibles, lower premiums, and lower co-pays.<\/p>\n\n\n\n<p id=\"block-49c60b3f-d75b-400a-8013-3ee470ba5926\">A health-contingent program is one where an employee must meet a health standard to qualify for a reward. A reward can be either an incentive OR a penalty.&nbsp; WellSteps strongly discourages the use of penalties. People respond better to positive rewards so that is what we promote.<\/p>\n\n\n\n<p id=\"block-1e3b5866-0bd7-446f-a23a-1516feab86e6\">Think about it this way. As a teenager, do you remember how much you disliked being told what to do? It turns out that teenagers &#8212; and most adults &#8212; don\u2019t like being told what to do! We dislike having privileges taken away even more.<\/p>\n\n\n\n<p id=\"block-9ba7dec4-3a06-4734-9f32-ce0e24ae63fc\">The punitive approach to employee wellness is not the happy option. We strongly recommend an approach that allows employee to earn a <em>positive<\/em> reward if they choose to. And yet, companies, in spite of their best intentions, often mess things up in their&nbsp;attempts to provide an employee wellness program. Let&#8217;s count the ways.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-how-do-health-contingent-wellness-programs-fit-into-broader-wellness-strategies\"><span class=\"ez-toc-section\" id=\"How_do_health-contingent_wellness_programs_fit_into_broader_wellness_strategies\"><\/span>How do health-contingent wellness programs fit into broader wellness strategies?<span class=\"ez-toc-section-end\"><\/span><\/h2>\n\n\n\n<p>A health-contingent wellness program, or &#8220;outcomes-based&#8221; program, uses biometric values such as blood glucose or blood pressure as the health standard. <\/p>\n\n\n\n<p>The problem is that too many companies <em>announce<\/em> the program standards but don&#8217;t actually do anything to help employees <em>change<\/em> the underlying behaviors and learn the skills necessary to meet the standards.<\/p>\n\n\n\n<p>Just because a company has a&nbsp; health-contingent wellness program does <em>not<\/em> mean they have a program that does any good! Remember that the program must be \u201c<a href=\"https:\/\/www.wellsteps.com\/blog\/2020\/01\/02\/health-contingent-wellness-program\/\">reasonably designed to promote health or prevent disease.<\/a>\u201d So how do you do that?<\/p>\n\n\n\n<p>An effective health-contingent wellness program does two things really well. First, it extends the program &#8220;reach&#8221; to as many employees as possible. And second, it provides an intervention that is robust enough to make an impact on health among the employees it reaches. After all, these employees are nearly always the high-cost ones. However, there is a problem with this logic. Employees tend to migrate across risk categories over time.<\/p>\n\n\n\n<p>Research shows that every year about 60% of the employees in the high risk group will be new. Employees with elevated health risks can move out of the high risk group by lowering their health risks OR by quitting their job OR by passing away (heaven forbid!). <\/p>\n\n\n\n<p>The point is, if the wellness programming focus is always on this year&#8217;s high risk employees, the programming will miss those employees whose unhealthy behaviors or risks will cause them to be in the high risk group next year (or the year after).<\/p>\n\n\n\n<p>Wellness programs focused on the high-risk only are chasing a moving target. A more effective approach is to focus on all employees across the entire risk spectrum. The chart below shows projected healthcare cost trends for this customer as compared to data from the Kaiser Family Foundation for other similarly sized organizations in their region. This is an example of how a health-contingent wellness program produced<a href=\"https:\/\/www.wellsteps.com\/blog\/2020\/01\/02\/outcomes-based-wellness-programs\/\" target=\"_blank\" rel=\"noreferrer noopener\"> great wellness outcomes.<\/a><\/p>\n\n\n\n<p>You can see that these two cost trends were nearly exactly the same until 2011. &nbsp;At this point, the client started with the <a href=\"https:\/\/www.wellsteps.com\/explore\/wellsteps-solution\/\">WellSteps wellness program<\/a>&nbsp;and&nbsp;you can see that their healthcare trend bent noticeably downward.
