Then Complete the 3 Wellness Activities

Complete the WellSteps Personal Health Assessment

Submit documentation of 2024 annual preventative physical exam*

Submit documentation of 2024 annual dental exam

Deadline to submit documentation is Dec. 13, 2024

 

 

* Annual preventative physical exams must be completed by a General Doctor, Family Practitioner, Internist, Primary Care Physician etc. on an annual basis. Diagnostic visits or follow up visits for an existing condition are NOT considered annual preventative physical exams.  “Kiosk” medical providers such as Retail Clinics cannot perform annual preventative physical exams as they generally do not complete comprehensive lab work, collect family medical history, compare year over year statistics and other important data that is needed to qualify. 

  • Acceptable documentation for your annual physical includes an Explanation of Benefit (EOB), summary of service, expanded receipt or a completed Screening Verification form.  Upload documentation in the WellSteps portal>Select Well Being Works Premium Discount. Please note: Screening date documentation must be within the timeframe of Jan. 1 - Dec.13, 2024.

If your spouse is covered on our insurance, both you and your spouse must complete all wellness activities to earn the discount.
