The Checklist to Change assesses the impact of the culture, physical environment, and workplace policies on employee health at your worksite.  A trained WellSteps guide will help you and your wellness committee implement the results from the Checklist to Change.  Complete the Checklist to Change below.

Physical Activity Environment
